Why are my monsters missing?

My monsters with a 0% miss chance are missing. (My speed is higher, no flying, no dodge). I have only seen this happen with my own attacks and have yet to witness the miracle miss benefit me.

Is there a base miss chance that's not posted anywhere? Incredibly infurating when you're trying to take out back line damage dealers with sneak and your monsters are missing for no reason.
